• Proof of Age: Birth certificate, Class 10 certificate, or any government-issued age proof.
• Educational Certificates: MBBS/MD/MS/DNB/MCh/MDS degree certificates, mark sheets, and relevant postgraduate qualification documents.
• Experience Certificates: Internship completion certificate, residency proof, or prior work experience letters (if applicable).
• Caste/Category Certificate: Valid SC/ST/OBC/EWS certificate issued by a competent authority, in prescribed format (if applicable).
• Passport-size Photographs: Recent colored photographs with white background, as per application guidelines.
• Identity Proof: Aadhaar card, Voter ID, Passport, or any valid government-issued ID.
• Registration Certificate: Medical Council of India (MCI) or State Medical Council registration proof.
